As the clinic continues to expand, the RN Care Coordinator will serve as a key connector across providers, specialties, patients, and families, helping ensure that each patientās journey from referral through follow-up is organized, responsive, and clinically informed.Ā

The RoleĀ
The Registered Nurse Care Coordinator serves as theĀ central clinical coordinatorĀ for a growing specialty clinic that supports adolescents and adults with complex, often lifelong conditions. This role requires strong assessment skills, exceptional organization, and the ability to coordinate across multiple disciplines in a fast-movingĀ ambulatory environment.Ā

The schedule isĀ Monday through Friday, 8:00 AM to 4:30 PM, withĀ no weekends, no holidays, and no call responsibilities. This position is currentlyĀ on-site five days per week.Ā

On anĀ average day, you may be reviewing incoming referrals, gathering outside records, triaging patient calls, preparing the care team for upcoming visits, and helping patients and families understandĀ the nextĀ steps in their care. You will work closely with attending physicians, nurse practitioners, an LPN, access team members, a registered dietitian, behavioral health specialist, genetic counselor, and specialty partners across UVA Health.Ā

Key responsibilities include:Ā
Managing referral intake and care coordinationĀ from initial referral through ongoing follow-up.Ā
Reviewing incoming referralsĀ and ensuring required documentation and outside medical records are available for provider review.Ā
Triaging patient calls and appointment requestsĀ using strong nursing judgment and patient assessment skills.Ā
Coordinating multidisciplinary care plansĀ for patients who may need involvementĀ fromĀ cardiology, physical therapy, occupational therapy, behavioral health, nutrition, genetics, ophthalmology, gastroenterology, and other specialty services.Ā
Supporting clinic preparationĀ by helping providers and ancillary teams stay informed, organized, and ready for complex visits.Ā
Responding to patient and family questionsĀ with clarity, compassion, and strong family-centered communication.Ā
Facilitating scheduling-related coordinationĀ and supporting provider workflows in a high-demand specialty clinic.Ā
Helping build future workflows and processesĀ as the Ehlers-Danlos Center continues to grow.Ā

This is an excellent fit for an RN who enjoysĀ buildingĀ new programs, thrives in a fast-growing clinical environment, and is comfortable partnering across multiple specialties. Pediatric nursing experience is strongly preferred, especially experience working with pediatric patients and families.Ā Ambulatory nursing experience in genetics, rheumatology,Ā neurologyĀ or complex care would be valuable to this role. Additionally, care coordination and Epic familiarityĀ isĀ preferred.Ā

MINIMUM REQUIREMENTS:
Education: Bachelor of Science in Nursing from an accredited nursing program.
Experience: 1 year of registered nursing experience required, 3 years of related and relevant registered nursing experience strongly preferred.
Specialty Board Certification strongly preferred
License/Certification: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification is required.

Job requires standing for prolonged periods, frequently traveling, and bending/stooping. Proficient communicative, auditory, and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids, and infectious diseases.
The pay range for this role is $85,820.80 - $113,401.60 annually. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.Benefits
