Registered Nurse Care Coordinator (RNCC) - Rheumatology and Infectious Disease, Children’s Hospital

As the Registered Nurse Care Coordinator for Pediatric Rheumatology and Infectious Disease, you will serve as a vital clinical connector across patients, families, providers, and interdisciplinary services. The Role 

In this role, you will help manage care for pediatric patients with autoimmune disease and infectious disease, supporting both clinic-based care and complex coordination needs that extend beyond the visit. The work is ideal for a confident nurse who enjoys thinking through clinical details, anticipating needs, and partnering closely with providers, clinical teammates, and other RN Care Coordinators. 

This is a Monday through Friday, 8:00 a.m. to 5:00 p.m., salaried clinic and office-based role, with some remote flexibility after onboarding. While there is patient interaction, much of the impact happens through coordination, triage, teaching, follow-through, and helping families navigate care with confidence. 

In an average day, you may: 

• Manage telephone triage for pediatric rheumatology and infectious disease patients. 

• Coordinate clinic visits, follow-up needs, treatments, and communication across services. 

• Provide patient and family teaching, including injection teaching and central line teaching and management. 

• Support administrative and clinical workflows such as forms, prior authorizations, and care documentation. 

• Partner with providers and the broader clinical team to ensure patients receive timely, coordinated, and high-quality care. 

• Help support evolving clinical priorities, including growth in rheumatology services and infectious disease coordination such as OPAT. 

• Use independent judgment, prioritization, and problem-solving to manage a defined patient population. 

This role is best suited for a nurse who is experienced, organized, collaborative, and comfortable working with a high degree of independence. Success in the first 6 to 12 months looks like confidently and independently managing the patient population while building trusted relationships with providers, families, and team members.
