Position Overview:
Admission Nurse Director's role is to coordinate the admission of residents to the facility in accordance with departmental goals, objectives, standards of performance and policies and procedure
- Grow census and case mix through responding to referrals and connecting with hospital case managers and social workers
- Develop and maintain business relationships with Discharge Planners, Case Managers, and Physicians to generate placement referrals
- Coordinate the admission of residents to the facility by communicating with other departments, external organizations, agencies and healthcare facilities. Ensure resident rooms are admission-ready
- Complete the admission packet with the residents and/or responsible parties
- Interview residents and obtain required information and signatures. Including discussion of room rates, billing procedures, visitor/guest privileges, restrictions, and resident care procedures as appropriate
- Keys information into computer systems, including EMR
Qualifications:
- Nursing education preferred (LPN or RN)
- Nurse License preferred (LPN or RN), but not required
- Requires a minimum of one (1) year of experience in a skilled nursing facility or similar healthcare setting, with knowledge of medical terminology, data entry, and the admission process. Experience should include coordinating admissions, completing admission documentation, obtaining required resident information and signatures, and communicating with healthcare facilities, residents, and responsible parties.
