Job DetailsLevel: ExperiencedJob Location: Main Campus - Inpatient - Jackson, MS 39216Position Type: Full TimeJob Shift: DayJob Category: Nurse The Case Manager (CM) coordinates inpatient rehabilitation care for a designated case load of patients and refers patients for post inpatient care CM is responsible for collaborating with all team members for identification of patient/family needs/goals, achieving economical utilization of services and planning discharge as well as follow-up care with the patient, family/caregiver/ and other facility The Case Managers serves as the IRF PAI Coordinator for his/her group of inpatients Shift Monday - Friday 8:00am - 4:30pm Qualifications Current Mississippi Registered Nurse License BSN, MS in Nursing and CRRN preferred Evidence of strong interpersonal skills, verbal and written communication skills Ability to physically assess and/or analyze patients with acute or rehabilitative requirements Demonstrates proficiency with utilizing standards of care, competency with nursing process and FIM scoring Knowledge of third-party reimbursement systems and utilization of referral services or ability to acquire knowledge Willingness to work flexible hours We Offer Competitive pay A comprehensive, flexible, benefit package to all full-time employees Medical and dental coverage beginning the first of the month following 30 days of employment  403b Retirement Life Insurance
