Key Responsibilities 

The Physician Services Population Health Manager will be responsible for developing and implementing strategies to enhance care coordination, manage chronic diseases, and improve overall community health. This role requires a strategic thinker with a strong background in success in value-based care arrangement and other advanced payment models, comprehensive knowledge on how to lead a successful clinically integrated network, strong comprehension with payor contracts. 

In this role you will be responsible for: 

  • Develop and implement a comprehensive population health strategy aligned with the organization’s mission and goals
  • Lead the Population Health team, providing direction, support, and professional development opportunities
  • Analyze risk through HCC/ICD10 coding and patient assessments to develop performance improvement strategies
  • Collaborate with coding specialists and healthcare providers to improve documentation practices
  • Design and oversee clinical programs aimed at improving patient outcomes, reducing healthcare costs, and enhancing patient satisfaction
  • Monitor and evaluate the effectiveness of population health initiatives and adjust strategies as needed
  • Utilize data analytics to identify trends, measure program effectiveness, and make data-driven decisions
  • Prepare and present reports on population health metrics to senior leadership and stakeholders
  • Work closely with healthcare providers, community organizations, and other stakeholders to coordinate care and address social determinants of health

Required Qualifications 

  • Minimum 5 years experience in healthcare management, with a focus on geriatric population health, care coordination, or chronic disease management
  • Experience with HCC/ICD10 coding and a strong understanding of its impact on healthcare reimbursement and quality metrics
  • Proven track record of developing and implementing successful population health programs
  • Understand levels of care defined through the Medicare benefit (Certified Home Health, Hospice, etc.)
  • Strong leadership and team management skills
  • Excellent analytical and problem-solving abilities
  • Effective communication and interpersonal skills
  • Ability to build and maintain relationships with diverse stakeholders
  • Proficiency in data analysis and the use of population health management software
  • Reliable transportation

Desired Qualifications 

  • Bachelor's degree in Healthcare Administration, Public Health, Nursing, or related field preferred

Hours 

  • 8:00 am – 5:00 pm, Monday through Friday 
Pay Range
$76,000—$91,000 USD
