Job DetailsJob Location: FLA-Jacksonville Area Command - Jacksonville, FL 32204Position Type: Full Time RegularSalary Range: $17.87 - $20.11 HourlyJob Category: Case ManagementSchedule/Hours: Monday, Tuesday, Thursday & Friday 8:30 to 5:00 located in Fernandina Beach, FL 322034  This position is responsible for:  Provides social service assistance to eligible clients in Nassau Co in the following areas: food, clothing, hygiene, transportation and financial assistance; interviews clients to determine need and eligibility to receive services; conducts site inspections and visits; prepares accurate and complete client records; enters data into various tracking spreadsheets and compiles periodic reports on same; refers clients to community agencies and maintains effective working relationships with the same; ensures compliance with program policies and procedures. Participates with Social Service Worker/Volunteer Coordinator and Program Director/Manager in ensuring smooth Center operations. Interacts with donors, partners, volunteers, clients and the general public, as necessary.  Key Responsibilities:  Interviews clients requesting financial assistance to determine client's eligibility based on program guidelines and restrictions; records client disposition and other pertinent information such as income, expenses, family and work history; completes client applications; photocopies pertinent documents such as income statements, utility bills, leases, etc. Contacts utility organizations and landlords to arrange and process financial assistance. Produces check requests, vendor documents and sends to applicable parties upon approval. Prepares client records in a timely manner ensuring all information is accurate and up-to-date; obtains signatures of clients as needed; accurately enters client data into HMIS database and applicable spreadsheets; maintains up-to-date social service case history files on all clients; maintains the confidentiality of same. Serves as primary client contact and coordinator for seasonal programs including Camp Keystone, Smart Start Back to School Program, Thanksgiving Baskets, Senior Angel Tree program, etc.; makes client contact; receives and processes applications and ensures clients of these programs are served effectively.  Physical Requirements and Working Conditions:  Work is performed in a busy open community center environment. Clientele served may not be well groomed. Work outside the center consists of visiting clients in their homes for inspections or case work follow-up.  Employee Benefits:  Paid Time Off Retirement Savings Plan Medical, Dental and more! QualificationsEDUCATION AND EXPERIENCE:  High school diploma or G.E.D.,AND  Two years’ experience working in a social or public service environment with experience assisting the public, OR  Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.   Licenses and Certifications:  Valid State Driver’s License.   *This position requires a level 2 background screening.
