Baptist Health is looking for a Registered Nurse, Case Manager at Baptist Medical Jacksonville - Downtown campus. This is a Full Time Day shift opportunity working Monday - Friday in the In-Patient Case Management Department at Baptist Jacksonville - Downtown campus.

Case Manager, Job Responsibilities:

• Care coordination for hospitalized patients.
• High risk screenings and assessments.
• Coordinating post-acute care services with proper documentation.
• Collaborating with multidisciplinary team members.

Experience Requirements:

• 1 year Case Management experience

Education & Credential  Requirements:

• Associate's Degree Nursing (ADN)
• Basic Life Support (BLS)
• Driver's Licensed (DMV)
• FL Active licensed Registered Nurse (RN)
