Requirements scheduling out until late July/early AugustSHIFT: DAYS : 8a-8pGulf Coast Medical Center/ Lee Memorial / Cape Coral/ Healthpark- Needs- - need 2-4 -Hospitalist Physician, ER patient assessments, admissions, daily rounding, orders placement, discharge planning, care coordination, EMR documentation of all care interventions, documentation of daily health status, changes and medication reconciliation.Onsite ? required to remain on campus for the full 12 hr shift ? required to work 7 days on / 7 daysStaring census 16 patients with up to 4 admissions for a total census of 18-20 patientsThere are operational leadership team members, office coordinators for assistance with schedule management and patient listsEMR- EPIC RequiredLee Health Privilege CriteriaREQUIREMENTS: Providers must have an active FL license, Clean background and clean NPDB, must be Board Certified and must meet all procedure log requirements to be booked- Applicants for initial appointment must be able to demonstrate active Internal/Hospitalist/Family Medicine practice for at least 24 inpatients in the past 12 months in an accredited hospital or healthcare facility (what the logs would need to show.) CASE LOGS : MUST be Primary Source and the report has to come from the facility with the facility name, provider?s name and dates of service or the range of dates. *** Must allow 30 days for Credentialing**
