What You’ll Do As an RN Clinical Consultant, you’ll combine clinical expertise, education, and relationship-building to strengthen connections throughout the healthcare community. You will: Serve as a clinical liaison and trusted partner to physicians, hospitals, care facilities, and community referral partners. Support appropriate referrals to Andwell’s home health, hospice, and palliative care services through clinical consultation and education. Build lasting, relationship-based partnerships grounded in trust, credibility, and clinical expertise. Provide clinical guidance regarding patient appropriateness, care transitions, eligibility, and available services. Collaborate closely with admissions, clinical, and care coordination teams to help create a smooth referral-to-admission experience. Develop and execute territory plans focused on relationship development, service awareness, and continuity of care. Work toward defined territory referral, service mix, and growth goals through clinically appropriate outreach strategies. Educate providers, patients, and families about Andwell services through in-services, meetings, and community outreach. Stay connected to referral activity, CRM documentation, and market trends within your assigned territory. Maintain current knowledge of Andwell services, regulatory requirements, and evolving care delivery models. Protect patient confidentiality and consistently uphold regulatory and organizational standards. What You’ll Bring You’re a nurse who understands that great care is about more than clinical knowledge—it’s also about listening, connecting, educating, and building trust. We’re looking for someone with: Current Registered Nurse (RN) license. Graduation from an accredited school of nursing; Bachelor’s degree in Nursing preferred. At least two (2) years of recent direct-care nursing experience in SNF, LTC, hospice, or a similar setting. Strong clinical assessment skills and the ability to translate patient needs into appropriate care recommendations. Experience collaborating with physicians, discharge planners, social workers, and interdisciplinary teams. A consultative communication style and the ability to educate partners about services, eligibility, and care pathways. A relationship-focused approach and genuine passion for supporting patients and families through transitions of care. Strong organizational skills and the ability to work independently while managing territory-based responsibilities. Proficiency with Microsoft Office and CRM platforms. Integrity, adaptability, sound clinical judgment, and a collaborative, team-oriented approach. A commitment to mission-driven, patient-centered care.
