MINIMUM REQUIREMENTS:         Must be a graduate of an accredited physician assistant training program; obtaining certification or certified by the National Commission on Certification of Physician Assistants and licensed by the Commonwealth of Pennsylvania, Bureau of    Professional and Occupational Affairs.    ESSENTIAL DUTIES:                       Working as a team, take medical histories, examine and treat patients, order and interpret laboratory tests and make diagnoses.   RESPONSIBILITIES:    Patient Care   The privacy of all medical records and other individually identifiable health information must be protected at all times. Information relating to a patient’s health care history, diagnosis, condition, treatment, or evaluation shall be considered individually identifiable health information. Confidentiality of this health information must be maintained at all times, and may only be disclosed with the express written consent of the patient.   Obtains a health and development history from patient or patient's parent.   Performs physical examinations, including developmental screening examinations on children.   Records pertinent data according to approved procedures in an organized medical record.   Performs clinical procedures, following written protocols established by the Medical Director; collaborates with the Medical Director in developing protocols.   Performs or initiates requests for commonly performed laboratory and/or diagnostic tests; follows up test results as needed.   Identifies normal and abnormal findings on histories, physical examinations, laboratory and diagnostic tests. Makes decisions regarding appropriate management and treatment of patients being seen for the initial evaluation of a problem or the follow-up evaluation of a problem previously diagnosed and stabilized.   Refers to specialists as needed, according to established protocols.  Consults with the appropriate physician on complex medical problems not covered by written protocols. Provides long-term clinical management and surveillance of adherence to medical regimen for  chronically ill persons; adjusts regimens within established protocols, recognizing when to refer the patient to the physician or health team member.   Initiates appropriate evaluation and management for emergency situations.   Provides counseling and education regarding health maintenance and common patient problems.   When appropriate, makes home visits to provide suitable evaluation and treatment for homebound patients.   B.  Administrative   With the Medical Director, develops patient care policies and procedures for clinic operations.   Advises the Medical Director of necessary supplies and equipment.   Assists in development of in-service training to alleviate deficiencies when necessary.   Advises the Medical Director of the Center's, and the patient populations' needs; assists in the planning and, when necessary and appropriate, implementation of methods to meet such needs.   Signs all necessary medical examination and insurance forms. C.  Community Service   Monitors the health care needs of the service area and attempts to fulfill them through the Medical Director.   Participates in community health education and projects as needed.   Acts as an advocate and liaison for the health center with the public, the medical community, the hospitals, the nursing homes, and with the local, state, and national societies as determined by the Medical Director.   Maintain membership in medical and specialty societies as needed.   Participates in after hour center activities as available.       QualificationsCornerstone Care is a Non-Profit, Federally Qualified Health Center with 14 locations and a mobile unit, serving communities throughout Southwestern Pennsylvania, and Northern West Virginia.  Our mission is to improve the health of our patients and all the residents of the communities we serve, with special concern for the medically underserved and low-income populations.
