Physical Therapist (PT) - Skilled Nursing Facility


Johnston, IA (Des Moines Metro Area)


Concept Rehab is seeking a Physical Therapist (PT) to join our rehabilitation team in a Skilled Nursing Facility (SNF) in Johnston, Iowa.


We have Full-Time, Part-Time, and PRN opportunities available for licensed Physical Therapists who are passionate about helping older adults regain mobility, independence, and quality of life following illness, injury, surgery, and hospitalization.

Requirements

Key Responsibilities

  • Evaluate patients following physician referral and establish individualized plans of care
  • Develop functional treatment goals that promote mobility and independence
  • Deliver skilled physical therapy interventions in accordance with evidence-based practice
  • Assess gait, balance, strength, endurance, transfers, and functional mobility
  • Supervise and collaborate with Physical Therapist Assistants (PTAs)
  • Communicate patient progress with physicians, nursing staff, families, and interdisciplinary team members
  • Participate in care conferences and discharge planning meetings
  • Educate patients and caregivers regarding safety, mobility, and home exercise programs
  • Maintain accurate and timely clinical documentation
  • Support quality outcomes and patient satisfaction initiatives

Qualifications

  • Graduate of an accredited Physical Therapy program
  • Current Iowa Physical Therapist license or eligibility for licensure
  • Strong communication and interpersonal skills
  • Ability to work collaboratively within an interdisciplinary rehabilitation team
  • Passion for serving older adults in a Skilled Nursing Facility and post-acute rehabilitation setting
  • New graduates are encouraged to apply
