RN Case Manager – Full-Time | Landmark Hospital of Joplin, Missouri
Are you an RN who loves connecting patients with the right resources at the right time? Landmark Hospital of Joplin is hiring a full-time RN Case Manager / Discharge Planner to join a close-knit, mission-driven team caring for medically complex patients in our LTACH/ARU setting.
What You'll Do
- Guide patients through an interdisciplinary care plan — from admission through discharge and safe return to the community
- Conduct initial and concurrent chart reviews for utilization management
- Complete biopsychosocial assessments and monitor patient response to treatment
- Partner with physicians to flag opportunities that reduce delays in extended stays
- Lead discharge planning, connecting patients to community resources and referral agencies
- Build strong relationships with external agencies and payor sources to ensure smooth transitions
- Collaborate with physicians to ensure accurate documentation of principal and secondary diagnoses
What You Bring
- Active RN license in Missouri
- Associate's or Bachelor's degree in Nursing (BSN preferred)
- 1–3 years of experience as a Case Manager and/or RN/Discharge Planner
- Current BLS Certification
- In-depth knowledge of system community resources and referral agencies
- Creativity and problem-solving skills
- Strong relationship-building and communication skills
Nice to Have
- LTAC or Acute Care Hospital experience
- Solid computer/EMR skills
