Able Home Healthcare is looking for a compassionate and skilled Home Health Registered Nurse to join our growing team in San Diego, CA. Responsibilities:
- Conduct comprehensive in-home patient assessments and develop individualized care plans
- Administer medications, wound care, IV therapy, and other skilled nursing treatments
- Monitor and document patient progress, vital signs, and changes in condition
- Coordinate care with physicians, therapists, and other members of the interdisciplinary team
- Educate patients and family members on disease management, medications, and home safety
- Ensure compliance with Medicare, Medicaid, and home health regulatory standards
- Maintain accurate and timely clinical documentation using electronic health records (EHR)
Requirements:
- Active California Registered Nurse (RN) license in good standing
- Minimum 1-2 years of clinical nursing experience; home health experience preferred
- Strong knowledge of Medicare home health guidelines and OASIS documentation
- Valid California driver's license, reliable transportation, and current auto insurance
- Excellent clinical assessment, critical thinking, and time management skills
- Proficiency with electronic health records (EHR) and point-of-care documentation
- CPR/BLS certification required
- Compassionate, self-motivated, and comfortable working independently in the field
