Clinical Pharmacist Coordinator

About the Role:

The Clinical Pharmacist Coordinator, under the Associate Director of Pharmacy Services, oversees pharmacist-led programs, medication use systems, and quality initiatives to ensure safe, effective, and evidence-based medication practices. They collaborate with providers, staff, and patients, integrating pharmaceutical care into interdisciplinary programs and ensuring compliance with regulations. The Pharmacy Services Department manages medication therapy for Hawaiʻi Island Community Health Center patients and serves as a key source of guidance on safe, appropriate, and cost-effective medication use.

Minimum Qualifications:

  • Required-BS Pharmacy Degree or a Doctor Pharmacy Degree from an accredited College/School of Pharmacy
  • Required-Licensed or eligible for licensure as a pharmacist in the State of HawaiÊ»i (must maintain licensure during employment tenure)
  • Preferred-Supervisory experience
  • Preferred-Completion of American Society of Health Systems Pharmacists (ASHP) accredited PGY-1 pharmacy residency or equivalent experience 
  • Preferred-Board Certification in either Ambulatory Care or Pharmacotherapy                                                                                                                                                                                                                                                       o A minimum of 2 years of pharmacy practice experience in lieu of board certification with the additional completion of the following:                                                                                                                                                 o Anticoagulation Forum Boot Camp or equivalent                                                                                                                                                                                                                                                                         o University of Washington HCV Course Modules (or equivalent)

Responsibilities:

  • Assists in the management of clinical programs led by the pharmacy department including, but not limited to Anticoagulation, Hepatitis C, Specialty Medications, and Pharmacist-led value-based care visits (including Medicare Annual Wellness Visits, Medication Therapy Management, etc.).                                                                                                                                                                                                                                               o Oversees the implementation of interdepartmental clinical workflows, ensuring proper integration into the electronic health record (EHR) system, and effective utilization by the healthcare team.                                     o Regularly evaluates and refines workflows for efficiency and effectiveness.                                                                                                                                                                                                                               o Provides leadership, guidance, and mentorship to pharmacists, nurses, and technicians regarding management of patient care that falls under pharmacy-led programs.                                                                           o Leads collaborative efforts with pharmacists, nurses, and other health center staff
  • Coordinates the Risk Evaluation and Mitigation Strategy (REMS) compliance programs for highrisk medications prescribed by HICHC providers (including enrollment, coordination of training, documentation of counseling and patient monitoring, as applicable).
  • Participates in strategic planning and discussions with external departments to promote collaboration

Skills:

This position requires strong interpersonal and communication skills, combined with expertise in medications, medical therapies, disease states, and pharmacokinetics. Experienced in clinical workflows, policies, and procedures, with added proficiency in technical equipment and information systems.
