• Collaborate: Partner with physicians, families, and referral sources to optimize patient outcomes.
• Innovate: Monitor quality metrics and lead performance improvement initiatives.

• Active RN license in Michigan
• Home Health experience required (OASIS experience strongly preferred)
• Leadership or supervisory experience is a plus
• Solid understanding of OASIS and home health regulations
• Strong clinical judgment and organizational skills
• Excellent communication and team‑building abilities
• Ability to multitask and manage competing priorities
• A passion for supporting nurses and delivering exceptional patient care
• Ability to work in the field as needed
