Job Description

Navigates patients through their diagnosis and treatment ensuring timely services. Meet with patient and families to provide emotional support, education and guidance to all patients. Provide medical information, education and review of reports and knowledge of community resources. Responsible for facilitating rapid scheduling and follow-up tracking of patients to ensure completion of their recommended treatment plan. Identify and problem solve any service gaps identified. Promote the Navigator role and prevention, detection and treatment awareness to the community through presentations and participation in related organizations. Coordinates and provides education for all newly diagnosed patients from our campus and outside community. Provides support to clinic nurse roles and maintains many additional job functions as needed.

Bachelor's Degree required or 5+ years experience in lieu of a degree.

When doing intake:

Responsible for identifying patient needs following initial referral, effectively facilitating transition of care from referring provider to first appointment, identifying and addressing testing needs including imaging, labs, and referrals as needed, and ensuring all patient information is up-to-date and correct for a meaningful first visit. This position will also be required to participate in patient experience initiatives to elevate care for this patient population and stakeholders.
  • Serve as the initial point of contact for new patient referrals as well as referring providers
  • Screen for appropriateness of referrals
  • Ensure appropriate and timely scheduling of new patient referrals
  • Facilitate comprehensive transition of care from referring provider to first appointment
  • Capture and organize new patient information
  • Identify and address patient barriers to care
  • Facilitate scheduling of imaging and lab testing appointments as well as additional referrals
  • Communicate and educate the care process to new patients
  • Act as an integral part of the shared decision-making and education process for new patients
  • Collaborate with team to respond to the needs of outside referring hospitals, clinics, and providers
  • Partner with the team in initiatives that elevate the patient care experience
  • Effectively leverage and facilitate adoption of electronic resources to elevate the patient experience
  • Partner with service line leadership to further develop and refine new patient intake processes
For Total Joint Replacement:

Navigate patients through their elective joint replacement from procedure scheduling through recovery. Meet with patient and families to provide emotional support, education, and guidance. Provide medical information, education, and review of reports and knowledge of community resources. Responsible for facilitating pre-op education class and follow-up tracking of patients to assess progress of recovery. Identify and address any clinical concerns and service gaps identified. Promote the Navigator role and prevention, detection and treatment awareness to the community through presentations and participation in related organizations. Support performance improvement efforts to maintain TJC Advanced Total Joint certification. Provides support and facilitates communication between inpatient and clinic staff and maintains many additional job functions as needed.
