Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program.  Our mission centers on "meeting individual needs through innovative care solutions," and they serve people across multiple counties in northeast Kansas. Visting Nurse Association Specializes in home hospice that provides compassionate, end-of-life care to patients in the comfort of their own homes. Their skilled nurses focus on managing pain and symptoms, supporting emotional and spiritual needs, and guiding both patients and families through the hospice journey. Working closely with an interdisciplinary team, the VNA ensures dignity, comfort, and quality of life during a patient’s final days while offering education, resources, and respite support to caregivers. Hospice Social Worker Provides specialized psychosocial support to patients and families facing life-limiting illness, addressing emotional, social, spiritual, and practical needs that affect comfort, coping, and quality of life. The role collaborates with the hospice interdisciplinary team to assess ongoing needs, develop and implement individualized plans of care, provide counseling related to anticipatory grief, loss, and end-of-life adjustment, and educate patients and families on the hospice philosophy. The position also connects families with community and financial resources, supports safe care environments, ensures regulatory and ethical compliance, and contributes to quality improvement, documentation, and community outreach, while maintaining professional boundaries and compassionate, patient-centered care throughout the hospice journey. QualificationsRequired Master’s degree in Social Work (MSW) Current licensure at the Master or Clinical level by the Kansas State Board of Behavioral Sciences Ability to assess psychosocial needs of hospice patients and families Knowledge of hospice philosophy and end-of-life care Ability to work effectively as part of an interdisciplinary team Strong communication, counseling, and documentation skills Ability to maintain confidentiality and professional boundaries Valid driver’s license and ability to travel to patient homes/facilities Ability to participate in occasional on-call   Preferred Clinical social work licensure (LSCSW) Previous experience in hospice, palliative care, home health, or healthcare settings Experience with grief, loss, and end-of-life counseling Knowledge of Medicaid, Medicare, and financial assistance programs Experience working with diverse cultural and socioeconomic populations Familiarity with electronic medical records (EMR) systems Experience facilitating groups, support meetings, or family conferences What’s in it for you?
