Job DetailsJob Location: 304 - JOL Hospice Katy - Katy, TX 77493Salary Range: $130.00 - $130.00 CommissionOverview: The Hospice Nurse Practitioner provides advanced nursing care to patients in end-of-life stages, working collaboratively with the interdisciplinary hospice team. This role includes conducting assessments, developing care plans, managing symptoms, and supporting patients and families with compassionate care. The Nurse Practitioner ensures compliance with hospice regulations and promotes quality of life for patients. Essential Functions: Conduct comprehensive assessments of hospice patients. Develop and implement individualized care plans in collaboration with the hospice team. Prescribe medications and treatments as appropriate under state regulations. Manage pain and symptoms to ensure patient comfort. Provide emotional and psychosocial support to patients and families. Document all care activities accurately and timely in the EMR system. Participate in interdisciplinary team meetings and contribute to care planning. Ensure compliance with hospice policies, procedures, and regulatory standards. Educate patients, families, and caregivers on disease progression and care expectations. Participate in on-call rotation and respond to urgent patient needs. QualificationsJob Qualifications: Education: Master’s degree in nursing; current Nurse Practitioner license in Texas. Certification in hospice and palliative care preferred. Experience: Minimum of 2 years of clinical experience; hospice or palliative care experience strongly preferred. Skills: Strong clinical assessment and decision-making skills. Excellent communication and interpersonal abilities. Proficiency with EMR systems and Microsoft Office. Ability to manage emotional demands of hospice care. Knowledge of Medicare and Medicaid hospice regulations. Environmental and Working Conditions: Works in patient homes, hospice facilities, and office environments. Ability to travel locally as needed. Flexible schedule including evenings and weekends. Exposure to various home environments and weather conditions. Physical and Mental Effort: Prolonged periods of driving and patient interaction. Ability to manage stressful situations in a calm and courteous manner. Requires working under some stressful conditions to meet patient needs and regulatory deadlines.
