Position Summary

St. Paul Elder Services is seeking a compassionate and motivated Physical Therapist to join our Rehabilitation team. This position is responsible for evaluating residents, developing individualized treatment plans, and providing evidence-based therapy services to help residents improve mobility, maximize independence, and achieve their highest level of function.

Working collaboratively with physicians, nursing staff, residents, families, and the interdisciplinary care team, the Physical Therapist will provide exceptional patient-centered care while ensuring compliance with professional standards, regulatory requirements, and organizational policies. This role also provides supervision and guidance to Physical Therapist Assistants (PTAs), rehabilitation support staff, students, and volunteers as appropriate.

Key Responsibilities

  • Evaluate residents and develop individualized treatment plans based on physician referrals and clinical assessments.
  • Implement evidence-based physical therapy interventions to improve mobility, strength, balance, endurance, and overall functional independence.
  • Establish measurable short- and long-term rehabilitation goals and modify treatment plans based on resident progress.
  • Complete timely and accurate clinical documentation in accordance with departmental policies and reimbursement requirements.
  • Collaborate with physicians, nursing staff, residents, families, and the interdisciplinary care team to coordinate care and discharge planning.
  • Educate residents, families, and caregivers on therapeutic exercises, mobility techniques, safety, and home exercise programs.
  • Monitor resident progress and modify treatment plans to maximize functional outcomes.
  • Participate in interdisciplinary care conferences and quality improvement initiatives.
  • Supervise Physical Therapist Assistants (PTAs), rehabilitation support staff, students, and volunteers in accordance with professional practice standards.
  • Promote resident safety by following infection prevention, emergency preparedness, and safety protocols.
  • Maintain productivity standards while delivering high-quality, resident-centered care.
  • Stay current with evidence-based rehabilitation practices through continuing education and professional development.

Required Qualifications

  • Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
  • Current Wisconsin Physical Therapist license in good standing.
  • Strong clinical assessment, treatment planning, and critical thinking skills.
  • Excellent communication, documentation, and interpersonal abilities.
  • Ability to work collaboratively within an interdisciplinary healthcare team.
  • Ability to safely lift, transfer, push, pull, and assist residents as needed (up to 50 pounds).
  • Commitment to providing compassionate, resident-centered care.

Preferred Qualifications

  • Experience in skilled nursing, long-term care, post-acute rehabilitation, assisted living, or geriatric care.
  • Experience supervising Physical Therapist Assistants (PTAs) or rehabilitation support staff.
  • Knowledge of Medicare documentation and reimbursement guidelines.
  • Membership in professional physical therapy organizations is encouraged.
