Overview

Join a mission-driven hospice team serving the Keene, Peterborough, and Charlestown, NH communities. As a Care Transitions Specialist, you'll build relationships with hospitals and healthcare facilities, support patients and families during critical transitions, and help ensure timely access to hospice care.

What You'll Do

  • Build relationships with hospitals, assisted living facilities, and skilled nursing facilities
  • Identify patients who may benefit from hospice services
  • Coordinate care plans with patients, families, nurses, physicians, and discharge planners
  • Educate referral partners and community members about hospice services
  • Support timely admissions and smooth transitions of care
  • Collaborate with the hospice team to improve patient outcomes
  • Track referral activity, goals, and outcomes
  • Stay current on healthcare trends and changes impacting patient care

Requirements:

  • Bachelor’s degree in Nursing or Social Work
  • At least 2 years of experience in a hospice setting.
