Join a mission-driven hospice team serving the Keene, Peterborough, and Charlestown, NH communities. As a Care Transitions Specialist, you'll build relationships with hospitals and healthcare facilities, support patients and families during critical transitions, and help ensure timely access to hospice care.

What You'll Do

Build relationships with hospitals, assisted living facilities, and skilled nursing facilities
Identify patients who may benefit from hospice services
Coordinate care plans with patients, families, nurses, physicians, and discharge planners
Educate referral partners and community members about hospice services
Support timely admissions and smooth transitions of care
Collaborate with the hospice team to improve patient outcomes
Track referral activity, goals, and outcomes
Stay current on healthcare trends and changes impacting patient care

Requirements:

Bachelor’s degree in Nursing or Social Work
At least 2 years of experience in a hospice setting.
Valid driver’s license, reliable vehicle, and auto insurance required
