The Wraparound Facilitator works in full partnership with child and family team members to develop a plan of care, oversees the implementation of the plan, identifies providers of services or family-based resources, assists in facilitating county consortium meetings, and monitors services for participants. Â The Wraparound Facilitator assures care is delivered in a manner consistent with strength-based, family centered, and culturally competent values, offers consultation and education to all providers regarding the values of the model, monitors progress toward treatment goals and assures that all necessary data of evaluation is gathered and recorded.
Education: Minimum Bachelor’s degree from an accredited university preferred.Â
Complete State Wraparound plan training within 18 months of hire.
Experience: A minimum of 2 or more years of one or combination of the following experience
- Clinical
- Case Management
- Skill Building
- Child Welfare
- Juvenile Justice
- Education in a K-12 school setting
Licensure/Certification: Must hold a valid state issued driver's license.
Full time, first shift, Monday- Friday, some days/hours could go into evening hours depending on availablilty to clients needs.
