What You’ll Do as an RN Case Manager
- Complete home health intake assessments, medication reconciliation, and care plans
- Collaborate with physicians, caregivers, and interdisciplinary teams
- Educate patients, families, and caregivers on disease management, safety, and recovery
- Maintain accurate clinical documentation and communication with staff
Qualifications
- Current RN license in Washington State
- Minimum 2 years nursing experience; home health experience preferred but not required
- CPR certification
- Licensed driver with reliable, insured vehicle
- Strong communication, clinical judgment, and patient-focused care skills
