How you’ll contribute
As a Nurse Navigator (RN), you will serve as a patient's guide through the healthcare system, helping coordinate care, remove barriers, and improve the overall patient experience.

Job Duties

Serve as the primary point of contact for patients and families
Coordinate appointments, testing, treatments, and referrals
Provide education about diagnoses, treatment options, and available resources
Assess barriers to care and connect patients with community resources and support services
Advocate for patients and helping ensure timely, appropriate care across the continuum
Provide hands-on patient care including but not limited to vitals, patient intake, in-clinic procedures
Cover other clinics as necessary
Other duties as assigned

Qualifications and requirements

Strong clinical judgment and critical thinking.
Ability to work independently in a fast-paced setting.

Clear communication with patients and care teams.

Qualifications

Standard ADN and BSN preferred
Outpatient Clinic preferred
ACLS and BCLS certifications required.
