Mobile Provider Services Nurse Practitioner (ARNP) – Home Health

Tri-Cities & Walla Walla, WA coverage area | Full-Time

Columbia River Home Health is seeking an experienced and compassionate Mobile Provider Services Nurse Practitioner (ARNP) to support patients across the Tri-Cities and Walla Walla communities. This role is ideal for an ARNP who values autonomy, relationship-based care, and improving quality of life through in-home medical oversight.

About the Role

As a Mobile Provider Services ARNP, you will deliver patient-centered care in home and community-based settings. You’ll collaborate closely with home health and hospice teams to manage complex conditions, support comfort-focused care, and ensure continuity across the care continuum.

This role averages 7–8 patient visits per day, allowing time for meaningful patient interactions, documentation, and care coordination.

Key Responsibilities

  • Monitor and manage chronic and acute disease states, including symptom control and wound care

  • Track and evaluate vital signs, symptoms, and treatment effectiveness

  • Prescribe and manage medications, addressing side effects and optimizing outcomes

  • Develop, implement, and update individualized care plans, including comfort-focused and palliative approaches

  • Provide patient and family education on disease management, self-care, and end-of-life options

  • Follow select Home Health patients requiring provider oversight

  • Conduct health risk assessments with an emphasis on quality of life

  • Address psychosocial and spiritual needs, supporting advance care planning discussions

  • Collaborate with physicians, specialists, home health clinicians, and hospice teams

  • Advocate for patient access to appropriate resources and services

  • Maintain accurate, timely, and compliant medical documentation

Qualifications

  • Active Washington State ARNP license with 1+ year of experience

  • Current Medicare Part B credentialing, or ability to obtain

  • Experience in home health, hospice, palliative care, or mobile medicine preferred

  • Strong clinical judgment, communication, and organizational skills

  • Valid driver’s license and insured vehicle in good working order
