Responsibilities:
- Deliver one-on-one skilled nursing care in home settings (wound care, catheter care, medication administration, symptom management)
- Perform assessments, evaluations, and skilled observation
- Educate patients and caregivers
- Collaborate with healthcare team for cohesive care
Requirements:
- Current WA LPN license
- BLS certification (AHA)
- Valid driver’s license and insured vehicle
- Home Health or Hospice experience preferred (not required)
