  • The social worker plays a vital role in supporting the emotional, psychosocial, and discharge planning needs of sub-acute residents.
  • Conducting comprehensive psychosocial assessments upon admission and updating them regularly.
  • The social worker actively participates in interdisciplinary care plan meetings and coordinates services for safe and timely discharges.
  • They provide counseling to residents and families, advocate for residents’ rights, and promote quality of life.
  • Acting as a liaison, the social worker fosters communication among residents, families, staff, and outside providers.
  • This role involves conflict resolution, maintaining compliant documentation, and helping residents adjust to facility life.
  • The social worker compiles social histories, prepares assessments and care plans, and revisits them quarterly or as needed.
  • They assist residents and families in understanding care goals and limitations, and educate staff on the psychosocial aspects impacting residents.
  • Address and resolve concerns using effective conflict resolution strategies
  • Maintain accurate and timely documentation in compliance with regulations
  • Collaborate with clinical staff, case managers, and external providers

Requirements

  • Minimum of a Bachelor’s degree in Social Work or a related field, Master’s degree in Social Work (MSW) preferred.
  • LMSW licensure, preferred
  • Experience in sub-acute or skilled nursing settings, strongly preferred.
  • Knowledge of Medicare/Medicaid guidelines
  • Strong organizational, communication, and interpersonal skills
  • Minimum of two years of experience in the social services field, preferably in a nursing facility, hospital or related agency.
  • Pleasant personality and ability to work with residents, families, and staff from various departments.
  • Proficient in Microsoft Office.
  • Ability to work well under pressure and be flexible.
  • Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, & sometimes challenging residents
  • Work well under pressure & willing to work harmoniously with all staff
  • Covid-19 vaccinations are encouraged but NOT MANDATED; we continue to offer COVID-19 vaccination based on eligibility guidance from CDC
