We are seeking a compassionate and motivated Nurse Practitioner or Physician Assistant to join our growing clinical team serving patients within skilled nursing care facilities in Lancaster, PA  Impact Health PC, a leading advanced practitioner group in skilled nursing facilities across the Northeast, that is dedicated to prioritizing patient care. Compensation and Benefits: Earning potential: $110,000–$200,000 (salary based on experience + easily achieved monthly bonuses) Flexible Schedule: Monday – Friday. No on-call, weekends, or holidays. Key Responsibilities: Deliver person-centered care using evidence-based practices Collaborate with interdisciplinary facility teams Conduct assessments, develop treatment plans, and manage follow-up care Educate patients, families, and staff Complete timely documentation (within 24 hours) Ensure compliance with state regulations in SNF settings QualificationsQualifications: Active license as a Nurse Practitioner (NP) or Physician Assistant (PA) Minimum 2 years of post-certification experience Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred Proficient with EHRs and clinical technology Strong communicator with a collaborative mindset
