Purpose: 

Responsible to triage patients over the phone or in the office to ensure prompt identification of patients with high-risk conditions. Information is presented to a physician for required action to be taken. Liaison between hospital, office and referring physicians. Population served based on scope of services in the department.

 

Responsibilities:

  1. Talks directly to patients on the telephone, provides a physician with necessary information and then directs them to emergency rooms or urgent care centers or to see their physician during office hours as instructed by a physician.
  2. Performs short evaluation of the patient situation to estimate severity of illness and/or injury including learning about chief complaint and, as possible, obtaining vital sign and mental status information.
  3. Confers with physician.
  4. Sends those with high-risk chief complaints such as chest pain to ER immediately as directed by physician.
  5. Sets up appointment for patients who do not need to go to ER but need to see a physician or arranges for an appointment scheduler to make the appointment.
  6. Process prescription renewals as directed by physician orders.
  7. Liaison between referring physicians calling office to speak with Cardiologist regarding mutual patients.
  8. Liaison between the hospitals and Cardiologists and disseminate requests for consults or transfers by rounding physicians. Enter these patients into the portal for rounding Cardiologists to see.
  9. Participate on the Emergency Response Team.
  10. Perform IV Lasix administration.
  11. Maintain and Model REACH Values (Respect, Excellence, Accountability, Compassion, Honor)
  12. Demonstrates regular, reliable and predictable attendance
  13. Performs other duties as required

 

Education and Experience Requirements:

  • Associates Degree in Nursing minimum, bachelor’s degree preferred.
  • Minimum 2-3 years of experience Nursing Triage or equivalent experience.   (Cardiology/ICU/ED/OR experience preferred.)

 

Minimum Knowledge, Skills and Abilities Requirements:

  • Knowledge of telephone-based clinical assessment techniques.
  • Knowledge of medical practice telephone triage protocols.
  • Knowledge of appropriate home health information for patients to follow until visit with physician, if they do not need an immediate ER visit.
  • Skill in using electronic medical records to check patient history.
  • Skill in using computerized medical information database during evaluation as guide to appropriate decision.
  • Skill in making triage decisions and responding quickly and calmly in emergency situations such as calling 911 and arranging for ambulance.
  • Skill in IV administration.

 

License, Registration, or Certification Requirements:

  • Valid NYS RN License
  • Valid ACLS Certification
  • Valid BLS Certification

Manual: Some manual skills/motor coord & finger dexterity

Occupational: Some occupational risk

Physical Effort: Medium to Heavy effort. May exert up to 35 lbs. force

Physical Environment: Some exposure to dirt, odors, noise, human waste, etc.
