Wage Range: $111,592.00 - $178,484.80 per year

Job Summary:
The Spine Nurse Navigator will enhance the patient experience by interfacing with various patient care teams and assist with the comprehensive care of Spine Center patients. Will work collaboratively with the Spine Center teams to ensure that the patients get seamless care for optimum outcomes. As a key member of EvergreenHealth’s Spine Center the Nurse Navigator serves as a patient-centered Orthopedic Spine Surgery resource from admit to discharge to outpatient follow-up. Provides patient education and assistance as needed with accessing clinical and supportive care services offered by EvergreenHealth and the community. After discharge, will remain a point of contact for families and patients. Functions on the multidisciplinary team as an advocate, educator, and counselor for Spine Center patients.

The Ortho and Spine Center of Excellence (OSCE) staff are required to complete specific orientation related to the orthopedic and spine care pathways for OSCE patients. All program staff will receive the department specific orientation that guides the care of OSCE patients and eight (8) hours of education per the annual education training plan that focuses on orthopedic and spine care.  

 

Primary Duties:

 

  1. Facilitates exemplary patient care including organization and management of policies, procedures, standards of care, and strategic objectives for Orthopedic Spine service line.
  2. Effectively communicates with Spine Program physician(s) and community providers to optimize and schedule services for patients.
  3. Assists in the development of and fosters comprehensive programs for the comprehensive Spine Surgery Program.
  4. Assists in the development of and fosters comprehensive Spine program.
  5. Interacts and facilitates care given by and communication between Spine care providers (nursing, physicians, physical therapist, pharmacy, radiology, case management, social work, etc.).
  6. Collaborates with the Medical Director and the Multidisciplinary Committee to ensure all policies and procedures necessary to care for spine patients are within the Orthopedic Practice Guidelines, Joint Commission Standards, and regulatory agencies.
  7. Assists with establishing and maintaining Dashboards/Scorecards in collaboration with the orthopedic multidisciplinary team, Spine Center medical director, and administrative executive for key service line metrics.
  8. Participates and establishes various workgroups to continue program development across the continuum of Spine care.
  9. Demonstrates the ability to gain the patient’s understanding and acceptance of the Spine Centers processes.
  10. Reviews the referred patient’s diagnosis, medical history, and physician disposition and effectively communicate this information to the patient.
  11. Assists physician in coordination of care of Spine patients with other health care professionals, then communicate the Physician disposition to the Primary Care or referring Physician.
  12. Follows up with patients and therapists in a timely manner to discuss the patient’s response to treatment, as well as obtain written progress reports.
  13. Communicates patient progress to the reviewing physician.
  14. Immediately communicates patient non- compliance to the reviewing physician.
  15. Communicates information about treatment results in symptom resolution to appropriate physician and helps determine future appointments with the Spine Center.
  16. Accurately provides on-going documentation of nurse navigation activities on each patient, including the time involved in these activities.
  17. Eliminates unnecessary delays in clinician reviews, including overseeing the collection of patient diagnostic studies.
  18. Performs other duties as assigned.

 

License, Certification, Education or Experience:

 

REQUIRED for the position: 

  • Bachelor’s in Nursing (BSN)
  • Licensed as a Registered Nurse in the State of Washington OR holding a multistate Nurse Licensure Compact (NLC) license
  • Minimum of 3 years full-time work experience in healthcare
  • Experience with direct customer service
  • Computer Skills
