POSITION OVERVIEW:
The Medical Social Worker provides emotional support, counseling, and resource coordination to patients facing terminal illness. This role plays a key part in the delivery of holistic care and ensures that patients' psychosocial needs are met with dignity and respect. A few position highlights include:
- Assist the patient and family with health related financial, social and emotional concerns and assist with effective coping skills.
- Provide social work services in accordance with the plan of care.
- Ensure that changes in the patient's clinical status are communicated to other healthcare team members and the agency in a timely manner.
- Educate the patient and family members and other team members on medical social work interventions and treatment plan.
- Perform care planning, transfer and discharge functions.
- Identify risks or threats to the patient's health in the patient's environment and promptly implement appropriate interventions to reduce risks. Communicates identification of risks/ threats to the patient, the physician, and other members of the healthcare team to coordinate care effectively.
THE MUST HAVES:
- Master's degree (MSW) or (BSW) from a school of Social Work accredited by the Council on Social Work Education.
- Must have a current and unencumbered Social Worker License in the state where services are rendered.
- Must have one (1) year of social work experience in a health care setting.
- Current CPR certification, driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
