POSITION SUMMARY: The Clinical Care Coordinator serves as an assistant to the agency Administrator, Executive Director, Director of Clinical Operations, and/or Patient Care Manager to ensure accuracy of patient electronic medical records information and documentation of hospice agency care processes. Provides oversight of daily agency clinical and office processes. 

 

ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING: 

  • Assists with phone calls to and from field staff, physician offices, referral sources, patients, and residential and healthcare facilities. Takes messages and receives physician verbal orders, as needed. Fields all phone calls and completes all follow up in a timely manner. Frequently monitors the status of incoming faxes, following up with the appropriate person, as needed.  

  • Maintains competency through assigned online courses and required in-services.  

  • Assists with visit verification, scheduling, and projection of visits, according to visit frequency orders and patient care needs.  

  • Assists with the Interdisciplinary Group (IDG) meeting process, including preparation for the meeting and completion of any required or requested reports.  

  • Provides skilled nursing services under the supervision of the RN Case Manager, as needed.  

  • Participates in hospice agency Quality Assessment Performance Improvement (QAPI) program, plan, and processes.  

  • Serves as an assistant to the agency leadership to ensure accurate data entry, updated patient electronic medical records, and oversight of daily agency office processes.  

  • Performs chart audits as assigned by agency leadership. Maintains audit competency.  

  • Maintains strict confidentiality of all patients.  

  • Maintains up to date knowledge, understanding, and skills related to the electronic medical record documentation system, all patient care system applications, and hospice agency operations applications such as Outlook, Teams, learning management system, and policy and procedure manuals. 

  • Performs other related duties as assigned.

QUALIFICATIONS & REQUIREMENTS FOR THE POSITION: 

  • Current Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) licensure in state of practice. 

  • Current CPR certification required. 

  • Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.

  • Limited portion (<30%) of daily assignments may require travel to client/resident/patient locations or other work sites, via car or public transportation. 

  • Work schedule may occasionally require evening, night, holiday, or weekend assignments.
