We provide high-quality, compassionate care to our residents, and we’re looking for dedicated RN MDS Coordinator who wants to make a real difference every day.  What You’ll Do: Oversee the assessment and documentation of resident needs, ensuring state and federal regulatory compliance, including Medicare and Medicaid. Optimize resident and facility reimbursement through data collection and clinical expertise.  Schedule Options Full Time (80 hours/pay period) Monday – Friday, regular business hours This role requires a flexible schedule, including the ability to work some weekends, evenings, and holidays based on facility needs. Job Responsibilities: Understand Medicare/Medicaid requirements Familiarity with MDS assessment tool and MDS coding Managing resident RAI assessments, care planning, and regulatory compliance Timely and accurate completion of MDS assessments Care plan development and updates Quality monitoring – including but not limited to resident chart audits Assist with implementation and training of restorative nursing program QualificationsRequirements: Must be currently licensed to practice in the state of Wisconsin as a Registered Nurse (RN).  2-4 years of RN experience preferred. RAC-CT Certification is beneficial, but not required. Experience working in Long-Term Care is beneficial, but not required.
