Position Summary

As a Hospice Community RN, you’ll provide compassionate, high-quality nursing care to patients in their homes and facility settings throughout the Granite VNA service area. You’ll partner closely with patients, families, providers, and the interdisciplinary team to develop individualized plans of care that support comfort, dignity, and quality of life.


What You’ll Do

  • Assess patient needs and develop individualized care plans 
  • Provide hands-on nursing care, education, and emotional support to patients and families 
  • Collaborate with physicians and interdisciplinary team members to coordinate care 
  • Document patient progress and communicate changes in condition 
  • Participate in care conferences, team initiatives, and ongoing education 
  • Supervise and support LPNs and LNAs assigned to patient care 
  • Travel throughout the service area to provide in-home patient visits

Requirements

  1. Graduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC).
  2. One to two years of recent general medical/surgical or skilled/acute care nursing experience.
