Job Description

 

THE POSITION IS PART-TIME (25 HOURS PER WEEK) MONDAY-THURSDAY IN-OFFICE AND FRIDAY WORK FROM HOME.  THE POSITION HAS THE POTENTIAL TO BECOME FULL-TIME IN JUNE 26'.  


The Community Hub Coordinator is responsible for organizing and implementation of the Community Care Hub for ReEntry in both Lafayette and Calcasieu parishes. They provide direct supervision to the Community Care Navigator for Reentry as well as the Community Hub Leads and Hub Intake Lead Specialists. They support the entire Community Hub process including the Community Hub Partner cases and the Community Hub Case Management aspects. The position reports directly to the Executive Director.

 

RESPONSIBILITIES:

1) General Responsibilities

  • Coordination and management of the entire process for Community Care Hub. This includes supervising referral processes, orientations, intake, community partnership referrals, and case management processes.
  • Provide supervision and guidance to Community Resource Specialists through the case management process.
  • Provide supervision for Community Hub Leads for the completion of follow-up activities for Hub Resource Partners.
  • Seek program and project expansion opportunities
    • Coordination, planning, and implementation of Community Care Hub.
    • Grant research, grant application submission, and grant administration.
    • Expand Hub partnerships and relationships.
  • Provide support and maintain relationships between Community Care Navigators and Hub Partners.
  • Responsible for all reporting requirements.



2) Communication Responsibilities:

  • Send and receive communications with Justice Partners
  • Send and receive communications with Hub Intake Lead
  • Send and receive communications with Community Hub Lead.
  • Send and receive communications with Hub Resource Partners
  • Participate in and remain current on operations, training opportunities, and news that can be relevant to the Justice and Community Care Hub.

 

3) Specific Influencing Responsibilities:

  • Participates in meetings and other local events to pursue implementation of justice initiatives. (LaRec)
  • Seeks opportunities to develop and implement policies and programs.
  • Align efforts with Beacon’s strategic plan.
  • Managing and implementing entirety of the project

 

4) Specific Learning Responsibilities:

  • Remain current on operations and news regarding partner agencies.
  • Develop and implement your individual learning plan.

 

 

5) Other Organization Responsibilities:

  • Responsible for implementation of assigned projects that carry out the organization’s mission.
  • Assist in fundraising and developing other resources necessary to support Beacon Community Connection, Inc.’s mission.
  • Responsible for the enhancement of Beacon Community Connection Inc.’s image by being active and visible in the community and by working closely with other professional, civic and private organizations.



PROFESSIONAL QUALIFICATIONS:

  • Bachelor’s degree preferred
  • Three or more years management experience preferred
  • Demonstrated ability to oversee and collaborate with staff.
  • Strong written and oral communication skills
  • Ability to interface and engage diverse employees, volunteers, donors and others.
