Home Health Physical Therapist Assistant (Laf-PRN)

Once a Physical Therapist has evaluated a patient and determined the course of action, the Physical Therapist Assistant often takes charge. This job requires someone who is comfortable following and giving orders. Someone with a careful eye who can modify exercises as needed and make sure the patient is doing them correctly. It’s a hands-on, patient-focused job with big rewards as you watch your patients increase function and mobility.

WHAT YOU WILL DO

• Provides physical therapy, including selection and use of assistive equipment.

• Reports patient progress and collaborates with evaluating therapist to implement therapy plan of care.

• Performs comprehensive physical therapy treatment and related follow-up services including discharge planning.

• Documents and submits patient reports within the established timeframe.

• Provides physical therapy according to standards of care defined by the state practice act and other professional and governing agencies.

• Collaborates with patient and other healthcare personnel or outside agencies regarding coordination of care.

• Trains and educates patients and family members or other support and documents understanding of treatment plan and goals.

• Provides services and treatments requiring physical therapy skills according to the written plan of care and the signed plan of treatment.

QUALIFICATIONS

• Associate's Degree- Accredited Physical Therapy Assistant Program- Required

• Physical Therapy Assistant (PTA)- State Licensing Board- Required

• Cardiopulmonary Resuscitation (CPR)- American Heart Association- Required

• Driver's License- Required

• Auto Insurance- Required

• 1 Year- Physical Therapist Assistant Experience- Required

• 1 Year- Home Healthcare Experience- Preferred
