Responsibilities

Assessing, planning, implementing, and evaluating care for individual patients
Provider will obtain detailed history from patients, perform physical examinations, and treat accordingly
Deliver high-quality adult and Adolescent care in collaboration with the in-office physician

Requirements & Skills

Board Certified
A Graduate of an accredited Nurse Practitioner program
Licensed to practice in the State of Indiana.
Previous experience in Occupational Medicine is a plus!
DOT Medical Certification preferred or willingness to obtain
Strong clinical background, with a focus on clinical excellence and patient satisfaction.
Must have strong written and verbal communication skills
Must have excellent multitasking, organizational, and computer skills

Monday – Friday
30 hours/week
