Responsibilities

Assessing, planning, implementing, and evaluating care for individual patients
Provider will obtain detailed history from patients, perform physical examinations, and treat accordingly
Deliver high quality Adult and Adolescent care in collaboration with the in-office physician

Requirements & Skills

Board Certified
A Graduate of an accredited Nurse Practitioner program
Licensed to practice in the State of Indiana.
Previous experience in Occupational Medicine is a plus!
DOT Medical Certification preferred or be willing to obtain
Strong clinical background, with a focus on clinical excellence and patient satisfaction.
Must have strong written and verbal communication skills
Must have excellent multi-tasking, organizational and computer skills

Employment Details

Monday – Friday
30-40 hours/week
