Case Manager – Short Term Rehab Â
Join our dedicated team at a critical access hospital specializing in short-term rehabilitation services. As a Case Manager, you will play a vital role in ensuring smooth patient transitions, from admission through discharge, while collaborating with external case managers and healthcare providers to deliver comprehensive rehabilitation services. Â
Key Responsibilities Â
- Collaborate with case managers from other facilities to coordinate patient admissions for short-term rehabilitation. Â
- Facilitate the admission process, ensuring all necessary documentation and assessments are completed accurately and efficiently. Â
- Oversee patient care during their stay, coordinating with medical, nursing, and therapy teams to meet individual needs. Â
- Develop and implement personalized discharge plans in collaboration with patients, families, and healthcare providers. Â
- Coordinate with home health agencies and community resources to ensure continuity of care post-discharge. Â
- Monitor patient progress and adjust care plans as needed to optimize recovery outcomes. Â
- Maintain accurate and timely documentation of patient interactions, care plans, and discharge summaries. Â
- Serve as a liaison between the hospital, patients, families, and external healthcare providers to promote effective communication. Â
Skills and Qualifications Â
- Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or related field; relevant experience may be considered in lieu of degree. Â
- Prior experience in case management, rehabilitation, or hospital settings preferred. Â
- Strong knowledge of healthcare coordination, discharge planning, and community resources. Â
- Excellent communication, organizational, and interpersonal skills. Â
- Ability to work collaboratively with multidisciplinary teams and external partners. Â
- Compassionate, patient-centered approach with strong problem-solving skills. Â
- Knowledge of healthcare regulations, documentation standards, and privacy laws. Â

Requirements
Education: Associates degree in healthcare or related field required. Bachelor’s Degree in nursing preferred. Must be a Registered Nurse in the state of Florida.
Experience: Minimum of five (5) years acute hospital nursing experience or three (3) to five (5) years acute hospital Case Management experience.
Skills: Strong organizational and interpersonal skills.
Knowledge: Knowledge of criteria for Medicare, Medicaid, HMO, and private insurance coverage. Knowledge of the continuum of care and utilization process.
Abilities: Ability to work independently, be attentive to detail, maintain a positive attitude, maintain confidentiality, complete work assignments timely, communicate effectively, be proactive, handle difficult situations in a professional manner and be able to document Case Management plans in a clear and concise manner. Ability to operate a computer and associated peripheral equipment.
