POSITION SUMMARY:
The Case Manager plays a vital role in supporting patients throughout their treatment journey by coordinating care and ensuring each individual receives appropriate services and support at every stage. This includes conducting assessments, developing individualized treatment and discharge plans, and collaborating closely with physicians, nurses, therapists, and community partners to address each patient’s mental health and social needs.
This role is essential in ensuring continuity of care by arranging follow-up services, connecting patients with housing resources, coordinating therapy, and linking individuals to community support systems after discharge. The Case Manager also serves as a key liaison between patients, families, and the treatment team, providing education, advocacy, and guidance to support recovery and stability.

PREFERRED KNOWLEDGE, SKILLS, AND/OR EXPERIENCE REQUIRED:
Education: A psychology major is preferred or nursing education
License: Nursing License, Social Worker License, &/or LPN
Experience: 1+ years coordinating utilization review and discharge planning activities
