QUALIFICATIONS:

  • High School graduate .
  • Ability to communicate accurately in writing. 
  • Ability to work well as a member of a team
  • Strong interest in providing quality support and care to persons with disabilities.  
  • Experience working in a program serving the developmentally disabled preferred.
  • Ability to transfer individuals with physical disabilities.
  • Possession of a clean, valid driver’s license is preferred.

ESSENTIAL RESPONSIBILITIES: 

  • Develop and maintain positive relationships with the persons with disabilities who receive Day Habilitation services. 
  • Implementation of individual’s Day Habilitation plan.
  • Assist with unloading and loading individuals to and from vans.  
  • Physically transfer participants during ADL and positioning activities.
  • Assist and train individuals in activities of daily living, including but not limited to: toileting, feeding, dressing. 
  • Administration of tube feedings/fluids as prescribed by medical staff.
  • Ensure that all record keeping and documentation completed is accurate and within required time frame.
  • Expeditiously reporting any sickness, injury or seizure to medical authorities.
  • Participate in cleaning and general maintenance of the Agency-owned property.
  • Participate in cleaning and maintenance of adaptive equipment and other personal possessions of consumers.
  • Transport and participate with individuals during Day Habilitation community activities.
  • Meetings with the Developmental Specialist and Community Inclusion Specialist to prepare for subsequent programming.
  • Participate in mandatory staff meetings and in-service training.
  • Perform any other related duties as directed by the Coordinator of Day Habilitation and/or designee.
