Your Role as a Physical Therapist

As a Physical Therapist with Concierge Home Care, you will deliver personalized, patient-centered care while leveraging advanced technology and AI-supported tools designed to streamline documentation and improve efficiency.

Key Responsibilities:

Travel to patients’ homes to provide individualized physical therapy services that promote recovery, mobility, and independence
Conduct comprehensive evaluations to assess patient conditions and rehabilitation needs
Develop and implement goal-oriented treatment plans in collaboration with patients, caregivers, and the interdisciplinary care team
Utilize AI-assisted documentation and charting tools to support timely, accurate note completion and faster turnaround times
Guide and supervise Physical Therapy Assistants to ensure high-quality, compliant care delivery

Qualifications

Unencumbered Florida Physical Therapy License(required)
Valid driver’s license, auto insurance, and reliable transportation (required)
Experience with OASIS documentation(preferred)
CPR Certification(required)
Ability to delegate and supervise Physical Therapy Assistants (preferred)
