As a Floating RN Case Manager, you step into different territories and help teams stay on track. You stabilize caseloads, support transitions, and make sure patients continue to receive consistent, high-quality care.
• You are trusted to step in, assess, and lead immediately.
• You gain exposure to multiple teams, patient populations, and care environments.
• You are not stuck in one routine, every week looks a little different.
• You still get the benefits of home health: 1:1 care and autonomy.
We operate alongside Interim, Lifespark, Adara, Aveanna, and Gentiva, but we invest heavily in nurses who can lead in the field and support growth across territories.
• Step into territories needing support due to census changes or coverage gaps.
• Provide skilled nursing care, assessments, and care planning.
• Complete OASIS visits and coordinate interdisciplinary care.
• Help maintain continuity and quality across teams.
• Educate patients and families while building trust quickly.
• Travel throughout the metro with flexibility and independence.
• Active Minnesota Registered Nurse (RN) license.
• Strong clinical judgment and ability to adapt quickly in new environments.
• Experience in home health preferred, but not required.
• Confident communicator and problem-solver.
• Reliable transportation and willingness to travel.
