About the Role
You're the steady hand families lean on at the hardest crossroads.
As a Hospice Intake Coordinator, you'll be the person who turns a referral into real care for patients and families facing the end-of-life journey. You'll review medical records and coordinate with physicians and healthcare providers to determine eligibility, then guide patients and families through what hospice means — answering their questions, easing their uncertainty, and offering steady support as they make one of the hardest decisions of their lives.
Behind the scenes, you'll manage the intake paperwork and consents with precision, and partner with nurses, social workers, and chaplains to coordinate a smooth transition into hospice care. You'll educate families on what to expect, stay ahead of compliance and documentation standards, and be ready to step in when a crisis or urgent need arises. It's a role built on accuracy and empathy in equal measure — you'll follow up to ensure care stays appropriate over time, manage referrals from healthcare partners, and keep growing your knowledge of hospice practices and regulations along the way.
What You Bring
1+ year of demonstrated customer service experience
1+ year of experience with medical terminology
Advanced computer skills, including proficiency with EHR systems and the Microsoft Suite
Bachelor's degree preferred

The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Colorado typically falls between $24.00 - $27.00 hourly with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state
