Salary: $95,000 a year

Social Worker Opportunity:

Full-time, permanent position
Schedule: Day shift; Monday-Friday

Provide social worker services by delivering behavioral health interventions as part of an interdisciplinary care team to improve patients’ physical health and psychosocial outcomes
Support individuals in managing chronic conditions through coaching sessions, medication and treatment plan adherence support, self-management education, goal setting, and facilitation of group classes

Social Worker Qualifications:

Social Worker must have or be willing to obtain LCSW or LMFT in the state of Missouri
Strong communication, organization, and multitasking skills with the ability to manage competing priorities and work effectively within a multidisciplinary team
BLS required upon hire
