Acute Case Manager

High Desert Medical Group is seeking a full time Acute Case Manager who will be responsible for evaluating inpatient medical records using SI/IS criteria, communicating with patients and families, facilitate discharge planning needs for the patients in the acute setting, skilled setting, and emergency rooms.

 Summary of Duties: 

• Perform inpatient reviews at each facility daily where patients of HDMG are hospitalized in an acute setting, weekly in the skilled setting, and time of visit for appropriate ER patients.
• Reviews medical record of each patient for SI, IS criteria.
• Communicate with hospitalist, attending physician and specialists regarding any utilization of hospital service issues that are noted.
• Communicates with care coordination staff regarding discharge planning needs.
• Identifies and initiates discharge planning needs along with hospital case management staff.
• Assist hospitalist and attending physician to identify continuum of care needs.
• Facilitate discharge plans.
• Attend Hospital Utilization Meetings and acute rounds as needed. 
• Participate in Case Manager's "on call" rotation.
• Perform other duties as assigned. 

The pay range for this position at commencement of employment is expected to be between $31.76 and $43.70/hourly, non-exempt; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience. The total compensation package for this position may also include other elements, including a sign-on bonus, restricted stock units, and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered. • High School diploma or equivalent.
• Graduate from an accredited LVN or RN Nursing Program, with current LVN or RN license.
• Current CPR certification.
• Minimum of 2 years clinical experience, Case Management experience preferred.
• Excellent written and verbal communication skills.
• Ability to handle and prioritize tasks in order to meet all given deadlines and productivity goals.
• Ability to responsibly handle matters of a confidential nature.
• Ability to work in a multi-task, high-demand environment.
