Home Hospice, Per Diem (as needed)

Very Flexible Hours

This specific role includes a new very flexible orientation. Some of the orientation can be done from home. Will require 5 full days of in person orientation. 

Works in collaboration with the Interdisciplinary Team (IDT) providing nursing services, assisting with care coordination, and assessing and addressing the ongoing needs of patients and caregivers. This role is out in the community visiting patients in various settings. 

PRN Requirements::

        At least 24 hrs./month.

        2 Saturdays per quarter

QUALIFICATIONS:

Registered Nurse, licensed in Pennsylvania. At least 6 months of recent experience preferred.
