Social Worker- Home Hospice

Full-Time

Day Shift (8am - 5pm)

Every 5th weekend is required

2 holidays per year required

This role works in collaboration with the Interdisciplinary Team to provide social work interventions, education, counseling and support services to address the identified psychosocial needs of patients and caregivers. MSW required, LSW or LCSW license must be obtained within 2 years of hire date. One year experience in the health care setting required. Must be able to complete 6 - 8 weeks of full time orientation.
