Primary Care Physician – Managed Care / Value-Based Care

Location: New Port Richey, FL

Position Overview

We are seeking a Primary Care Physician to join a growing, senior-focused primary care practice dedicated to delivering high-quality, relationship-based care for adults age 65 and older. This is an excellent opportunity for a physician who enjoys practicing medicine in a value-based, managed care environment with smaller patient panels, longer appointment times, and robust clinical support.

The practice emphasizes personalized care, preventive medicine, chronic disease management, and strong physician-patient relationships. Physicians are supported by an experienced care team, allowing them to focus on patient outcomes rather than high-volume visits.

Responsibilities

• Provide comprehensive primary care services to Medicare Advantage and senior patients age 65+
  
• Perform thorough assessments and develop individualized care plans
  
• Manage chronic conditions, preventive care, and acute medical concerns
  
• Deliver patient-centered care within a managed care/value-based care model
  
• Collaborate with physicians, specialists, and interdisciplinary care teams to coordinate treatment plans
  
• Identify and close care gaps while supporting quality and outcome initiatives
  
• Educate patients and caregivers regarding disease prevention, healthy lifestyle choices, and long-term wellness
  
• Build strong relationships with patients and families through compassionate, personalized care
  
• Maintain accurate and timely documentation within the EMR
  

Schedule

• Monday – Friday
  
• 8:00 AM – 5:00 PM
  

Qualifications

• MD or DO degree from an accredited medical school
  
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics
  
• Experience in Managed Care, Value-Based Care, or Medicare Advantage preferred
  
• New Graduates Welcome
  
• Florida Medical License preferred or eligible
  
• ACN licensed accepted​
  
• Geriatric care experience preferred
  
• Strong communication and patient engagement skills
  
• Spanish-speaking ability preferred for one opening
