Position Summary: Â
The physical therapist will accurately assess, differentially diagnosis, and identify deficits in a patient’s ability to move and perform functional activities in their daily lives.  They develop an individualized treatment plan with patient-centered goals to reduce pain, restore function, and prevent disability and work with the individual to prevent the loss of mobility.
Essential Functions and Responsibilities:
1. Â Â Performs patient evaluations through the process of obtaining history, performing relevant systems review, and selecting and administering specific tests and measures to obtain data.
2. Â Â Establishes an individualized plan of care and selects treatment techniques that addresses the specific impairments identified in the evaluation and in accordance with physician orders.
3. Â Â Conducts ongoing reassessment of the patient and their response to treatment.
4. Â Â Initiates and manages the discharge plan beginning at the first visit and continuing throughout treatment.
5. Â Â Supervises and delegates to ancillary staff and students consistent with state laws and department policy.
6. Â Â Completes legible, accurate and clear documentation within the established timeframe identified in the department policy and procedure and in accordance with accreditation requirements and insurance regulations.
•   Graduate of an accredited program of physical therapy.
•   Current state Board of Physical Therapy licensure. Â
•   BLS certification.
•   One year of physical therapist experience.

Required:
•   Valid driver’s license, proof of valid auto insurance, and maintain a driving record in accordance with McLaren’s corporate driving policy and eligibility guidelines.
