About the Role

At Sierra Health Care, social work isn't a support service — it's a cornerstone of the care we provide. As a Licensed Clinical Social Worker, you'll work alongside patients and families navigating some of life's most complex moments, bringing clinical expertise, emotional steadiness, and genuine compassion into their homes. Sierra has been part of the Las Cruces community since 1982, and our social workers are among the most trusted members of our interdisciplinary team.

Key Responsibilities

  • Conduct mental health and behavioral health assessments under a physician's plan of care
  • Provide counseling, psychotherapy, and psychosocial support to patients and families
  • Support grief counseling, end-of-life decision-making, and advanced care planning
  • Contribute to diagnosis and care planning for patients with complex emotional and social needs
  • Collaborate with the interdisciplinary team to coordinate care across clinical, emotional, and social domains
  • Document all interventions, assessments, and care coordination per regulatory and agency standards
  • Participate in QAPI activities, in-service training, and patient care planning meetings

Other Duties:

  • Provide care and services in accordance with infection control, safety, and risk prevention protocols
  • Accurately document clinical interventions, care coordination, and communication in a timely manner
  • Participate in interdisciplinary meetings, case reviews, and continuing education opportunities
  • Maintain compliance with HIPAA and all company privacy and confidentiality standards
  • Represent the organization positively and professionally in patients’ homes and community settings
  • Work independently with minimal daily oversight and demonstrate initiative in completing responsibilities
  • Provide courteous, timely, and professional customer service to patients, families, and partners
  • Exhibit adaptability to changing responsibilities and flexibility in completing assignments
  • Demonstrate consistent and effective use of organizational technology systems and tools required for the role, including communication, documentation, productivity, and data management platforms
  • Adhere to all organizational policies, including job descriptions, mission, and the Employee Handbook
  • Communicate and interact professionally and respectfully with others to support team goals
  • Perform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the company


Requirements

 

What We're Looking For

  • Master's degree in Social Work from a CSWE-accredited program
  • Current, unencumbered LCSW license in New Mexico
  • Two or more years of clinical social work experience preferred
  • Demonstrated competency in behavioral health, family dynamics, and counseling techniques
  • Strong documentation and communication skills
  • EMR proficiency and ability to learn hospice and home health documentation systems
  • Reliable transportation, valid driver's license, and auto insurance for home visits
